What You Need to Obtain a Filing
Depending on the filing or certificate of insurance (COI) that you need, you must provide specific information to complete your filing request.
Information you may need to provide for us to process a filing can include:
- Doing Business As (DBA) name
- Phone number
- Social Security number
- Names of any drivers to be listed
- Business address
Trucking businesses may also need to supply:
- Docket Number (MC#)
- Department of Transportation Number
- FEIN # for OS32 and Perm 17
Required Insurance Coverages
To obtain a filing, you also must meet the associated insurance requirements by carrying the required insurance coverage with at least the minimum limits.
